Uniquely Compelling Flexibility
Designed to seamlessly complement an employer medical plan, our GAP program comes with a range of unique riders. Plans are available with employee- and/or employer-paid options, with age-banded or composite rates, and also cover outpatient prescription drugs, ambulance and doctor office visits.
Customize Coverage
Base plans can include inpatient benefits only or combine inpatient and outpatient benefits.
Inpatient Benefits
Covers inpatient hospital stays, inpatient surgeries and physician’s inpatient charges if these expenses are covered under the insured’s medical plan.
Benefits are determined by the employer based on the employer’s major medical plan. Available coverage amounts:

Outpatient Benefits
Coverage includes outpatient treatment of injury and sickness. Treatment must be performed in an emergency room, urgent care facility, free-standing facility, ambulatory surgical center and/or outpatient hospital facility.
Available coverage amounts, as determined by a percentage (up to %100) of the Inpatient Benefits, are:
